Immigration Physcial
US Citizenship and Immigration Services requires immigration physical as part of the process to become a lawful permanent resident. You will need to bring the following documents with you to the office:
● Government-issued photo ID (driver’s license or passport)
● Form I-693 (Report of Medical Examination)
● Any vaccination records
Dr. Kaw performs a comprehensive physical exam. He conducts a TB (tuberculosis) screening test, or recommends a chest X-ray to look for signs of tuberculosis. You may also need blood tests.
Most of these services are conveniently offered onsite at the office. US immigration laws also require you to be assessed for contagious diseases, including measles, mumps, and more. That is why it is essential to bring your immunization record.
Dr. Kaw completes your I-693 form once the physical and required lab tests are completed. The turnaround time is about 5 to 7 business days or less.

School Physical
A back-to-school physical focuses on preventive health, such as immunizations and screening tests. Dr. Kaw ensures your child or teen has up-to-date vaccinations and completes any paperwork so that they are ready for the upcoming school year.
Well-Woman Physical
Dr. Kaw focuses on comprehensive preventive care for women’s health. He recommends
cancer screenings, such as routine Pap smears to screen for cervical cancer, mammogram to screen for breast cancer; also screens for common sexually-transmitted diseases, if applicable. He performs a physical exam, including a breast exam and pelvic exam.
Sports Physical
A sports physical helps to ensure your child is healthy enough to participate in sports. Dr. Kaw takes a complete medical history, asking about health issues that may affect your child’s ability to play sports safely.
Next, Dr. Kaw performs a comprehensive physical exam. If your child is healthy, Dr. Kaw signs the necessary paperwork.
